If you are in CT or nearby NYC, send your passport and copy of the passport documents to NYC consulate along with necessary fee. They will send you the certify that, else you can go to NYC India consulate and get the documents certified, all details are available on NYC Indian consulate website. I did this procedure twice 3 years back from India NYC consulate, [for apply ITIN for my wife and daugther] and another instance is to get certified copy of my passport to get passport of my daughter made. Just go to NYC India consulate website and all are documented there. IRS returns your certified copy along with your ITIN. Thanks
